Disability Services

Your Confidential File

 

Disability Services (ODS) establishes a file for each student once the student applies for services. The file is designated in one of three ways:

 

  • Potential Student: Before you begin services and sign your first contract, ODS starts a file with all the information needed to put you in the system.
  • Active Student: Students currently using services are active. To keep your file active, you must submit a request for services EVERY semester including all three summer terms.
  • Inactive Student: If you do not request services, your file will be made inactive. Remember, you cannot use Priority Registration or any accommodations if your file is inactive.

 

In your file, ODS keeps information based on each contact you make to discuss services with our staff, as well as copies of signed contracts and service letters. We also keep copies of alternative tests that have been taken during the present term. Any information sent out on your behalf, or information received by our department is kept in your file.

 

Your file and status helps us to ensure your needs are being met. If you have questions about the information in your file, please feel free to speak with the professional staff, which is the Office Manager, the Director, or the Accommodations Coordinator.

 

 

Confidentiality

All disability documentation presented to ODS will be kept in a confidential file. The access to this information is restricted to office personnel only. ODS will not release any information without a completed Release of Information form, identifying the person(s) with whom to share information. The Release of Information form must be in writing and completed in the presence of a ODS staff member. The Release of Information form must be completed for each incidence.
