Requestable Services
To receive notetakers, you need to submit a service request form every term listing all of the classes in which you want notes. We try to match you with a student that is already in the same class. It has been our experience that notes are more complete when the notetaker has read the background material and has an idea of which concepts are important to the professor.
It is important to note that receiving notes is not a substitute for attending class. Unless your absence is related to your disability, you will not receive notes if you miss class. If you do miss class, it is your responsibility to obtain notes from another student.
Please let us know if you are having problems with your notetaker. If we know of specific problems, we can work with your notetaker to correct the problems. Your notetaker may be taking notes in several classes and may not be able to provide your copies of the notes immediately after class. It is critically important that you check your mailbox regularly, at least once a week. Your mailbox is more than a place to deposit your notes; it is the mechanism that we use to communicate with you. Notetakers also have mailboxes so that you can communicate with them.
Requesting Alternative Testing:
In order to receive alternative testing, you need to fill out a service request form each term and submit an Alternative Test Arrangements form (pictured below). It is very important that you completely fill out each form. After you have filled out the first section, take the form to your professor and ask them to complete the next section. Please schedule your exam so that it can be completed between 8 a.m. and 5 p.m. Monday through Friday. This form must be filled out completely and returned to our office no later than seven days prior to the time you plan to take the exam. If the Office of Disability Services does not receive your form at least seven days in advance, you will need to take the exam with the rest of your class, or make other arrangements with your professor. It is your responsibility to make sure that our office has your forms at least seven days in advance. If the form is not correctly completed, it will be returned to you. We recommend that you fill out your forms as soon as you have your syllabus.
Please be on time for your exam. If you are late for your scheduled exam, you will not be given additional time to compensate for your tardiness. You will only be allowed to take items into the exam room that are approved by the professor. Depending upon the type of alternative testing you are approved for, you may use a laptop or have a proctor to read, or scribe your exam. If for some reason you are unable to attend your exam, please notify the Office of Disability Services. It is your responsibility to make alternate arrangements with your professor to take the exam. If you have a medical reason related to your disability that prevents you from taking your exam, please notify our office.

If you need a sign language interpreter, you need to request them through our office. The form that you use to request an interpreter depends upon the type of activity. To request interpreters for classes, you need to fill out a Service Request Form requesting an interpreter at the beginning of each term. Please submit your service request form as early as possible so that we can arrange both interpreters and note takers for your classes. Remember, if you add or drop a class during the term, you need to fill out a peach-colored Add/Drop Form.
If you need an interpreter for an event sponsored by an organization on campus or to meet with your professor or another office on campus, you need to fill out a Miscellaneous Interpreter Request Form (pictured below). We need at least 48 hours notice to be able to provide an interpreter. However, the earlier you notify us that you need an interpreter, the easier it is to arrange. If the event is scheduled to last all day or overnight, we need a minimum of one-week notice. Please fill out this form as completely as possible and include your e-mail address.
