Please send questions about this form to global@wou.edu or call 503.838.8425 Request New I-20 or DS-2019Please submit the form below to request a new I-20 due to having lost or damaged your document or other reason you indicate. After you submit this form, an International Student Advisor will review your records, update your SEVIS record and print a new I-20 for you to pick up at Maaske Hall. Remember to sign the new I-20 as soon as you pick it up. Thank you.
