Forms for graduate studies This office maintains the academic record for each graduate student at Western Oregon University while he/she is actively taking course work in his or her master’s degree program. Once a student graduates or withdraws the file is sent to the University Registrar for safekeeping.

 

All forms available through this web site may be printed and filled out by hand. Forms must be mailed or hand delivered to the Graduate Office with appropriate signatures before they considered official. In some cases, you will have to send a form to your advisor and request that your advisor send the signed form to the Graduate Office.

 

Forms that require an advisor or other institutional signature need to be printed and delivered to the appropriate office for signing. Some forms that only require a student signature can have a typed (electronic) signature as long as the box marked "By typing your name below, you agree that this as valid as your signature" is checked. Any of these forms can be e-mailed to the appropriate destination as long as the person emailing the form recognizes the following: When sending sensitive information (e.g. social security number or other personal information) from a wou.edu address to another wou.edu address, the attachment is sent through a secure, encrypted server and should be viewed as protected. If, however, you send sensitive information from a non-wou.edu email address, we cannot guarantee the safety of your information and advise against doing this. For students who do not feel comfortable using an electronic signature, please fill out the form electronically, print it out, and then sign by hand.

Admission

Before you can become a candidate for any WOU graduate program, you must apply to the university, and to the department which administers your graduate program.

    Each master’s degree program has its own application form and requirements. Information and forms are available from the program's Web site, deparment offices, and the Graduate Office.

Plan of Study

Obtain this form from your advisor. Each master’s degree program has a unique Plan of Study. A Plan of Study must be on file in the Graduate Office in order to receive federal financial aid.

  • This form can only be filled out and submitted by an instructor who has taught a course that you have taken as part of your master’s degree. The graduate student/candidate should completely fill out the top portion of this form before giving the form to the instructor.

Change of program From time to time students find that the Plan of Study must be changed. Two forms can be used to accomplish a change:

  • Graduate change of program request
    This form should only be used to make direct course substitutions for courses that are electives in a degree program.
  • Academic or licensure petition
    Petition to waive, substitute, or demonstrate competency for a course required in an education and/or licensure graduate program. When a change is requested for a requirement within an approved teacher license program the reason for the change must be clearly indicated. This is a requirement of the Oregon Teacher’s Standards and Practices Commission (TSPC). As a general guideline, all graduate students in a teacher license program should use this form for all degree modifications and provide a rational for the change requested.

  • Master’s degree final evaluation report
    In order to complete a Thesis, Professional Project, Field Study, Portfolio, or Advance Proficiency Documentation (Continuing Teacher License), an oral examination is required. The results of the oral examination are reported on this form.
  • Application for completion of a master’s degree
    Graduate students must apply for graduation and/or comprehensive examinations during the term before the term the student wishes to complete their degree. In all cases, applications must be received at least 10-15 weeks before the expected date of degree completion. Early applications will be accepted.
