Before You Begin
When to Use This Form
This form is specifically for re-ordering documents which have already been created by this office in the past. If you are planning a new document or extensive revisions to an existing document, you should make an appointment with Denise Visuaño or Stewart Gilbert to ensure the best outcome for your project.

Who to Consult
Generally speaking, if your document is to be printed, make an appointment with Denise at 503-838-8349; if it is for electronic distribution or for the web site, make an appointment with Stewart at 503-838-8761. (We both do both types of work, but tend to allocate jobs based on timing and type of work.) Our fax number is 503-838-8164.

Form Instructions

Complete Information
Excluding the notes fields, please provide all information requested. Complete information helps us find your earlier project, update it and deliver it to you in a timely fashion.

Delivery Dates
The earliest date for delivery of any project is three weeks from today. If there are special circumstances regarding the delivery date, please note them in the project notes field.

Projects
There is space for up to three projects to be grouped under one order. For example, you may need a program, a poster and a postcard for one event. In this case, each of the three projects would use the same art, styles, font faces, and have similar design. For projects with disparate designs, please submit individual orders for each.

Order Completion
When complete, submit your order with the “Submit Order” button below. Your order will be converted into an e-mail and delivered to both Denise and David. (If you see an interim screen asking you to send e-mail, please click the button to complete your order.)
